Registration & Immunizations

If you are new to the area or have a child entering kindergarten for the first time, we welcome you! You will find all the information you need to register your child at one of Eloy’s fine schools right here on this page.

Registration

To register your child in the Eloy Elementary School District, please stop by our district office. When you arrive, be sure to have the following documents with you.

Immunizations

Arizona state law requires every parent to complete an immunization history for his or her child at the time of enrollment. The immunization record must show the child’s name, date of birth, and the month, day, and year of all immunizations. All students enrolled in Eloy ESD schools must be current on immunizations unless they have a signed exemption form on file in the health office. We reserve the right to exclude exempted children from school should there be an outbreak of any communicable diseases.
